From your initial call, A Place At Home will discover what your personal concerns and goals are and where we can help. We will complete a basic needs assessment and set up an in-home consultation with you and any family members that want to be involved. We will work with you and your healthcare team to develop an initial Care Plan to ensure your care needs are being effectively addressed, and we will answer any questions you may have concerning care. We will ensure you’re aware of any financial resources or benefits available to you to help cover the cost of care. Once services start, our team is available to you 24/7 should any needs arise.

Personalized Care Plan
We will make detailed recommendations and develop a personalized care plan tailored to your loved one’s specific needs. We will arrange a follow-up meeting to review the Care Plan with you to ensure communication is open and any modifications needed can be addressed.

Matched with Caregivers
Once the Care Plan has been reviewed and approved, we select the appropriate and most qualified caregivers to meet your specific needs. We evaluate the services you need, your interests and personality and find the best caregiver match possible. Finding the best match for you is a priority for us and we are always responsive to your thoughts and feedback about members of your care team.

Care Monitoring
Through client and caregiver surveys, the quality of care provided is routinely monitored and evaluated. Your local A Place At Home office takes your feedback very seriously and will make adjustments accordingly. Caregivers keep a record of the services they provide through our online portal which is available at any time to the family. Our billing system also provides a detailed description of the services provided.
