This collaboration is part of the Centers for Medicare &amp; Medicaid Services (CMS) GUIDE Model, a nationwide initiative to improve support for individuals living with dementia and their families.<\/p>\n<p data-start=\"539\" data-end=\"775\">Through this program, A Place At Home can now provide Medicare-covered in-home respite care, giving local caregivers a much-needed break while ensuring loved ones with dementia receive compassionate, professional care.<\/p>\n<h2 data-start=\"782\" data-end=\"830\">What This Means for Families in Yorba Linda<\/h2>\n<p data-start=\"832\" data-end=\"1040\">Caring for someone with dementia is not only emotionally challenging, but also physically and financially overwhelming. By combining Medicare-covered respite hours with 24\/7 access to trained nurses, we\u2019re not only expanding care, but we\u2019re also helping caregivers breathe, reset, and know they\u2019re not alone.\u201d<\/p>\n<\/blockquote>\n<h2 data-start=\"2849\" data-end=\"2875\">How the Program Works<\/h2>\n<ul data-start=\"2877\" data-end=\"3401\">\n<li data-start=\"2877\" data-end=\"3078\">\n<p data-start=\"2879\" data-end=\"3078\">Local franchise owners, including A Place At Home Yorba Linda, will <strong data-start=\"2947\" data-end=\"2973\">refer eligible clients<\/strong> with a dementia diagnosis (or suspected diagnosis) enrolled in Medicare Parts A and B to <strong data-start=\"3063\" data-end=\"3075\">PocketRN<\/strong>.<\/p>\n<\/li>\n<li data-start=\"3079\" data-end=\"3154\">\n<p data-start=\"3081\" data-end=\"3154\">PocketRN will conduct required assessments in line with CMS guidelines.<\/p>\n<\/li>\n<li data-start=\"3155\" data-end=\"3274\">\n<p data-start=\"3157\" data-end=\"3274\">Once approved, A Place At Home Yorba Linda caregivers will perform home safety evaluations and begin care delivery.<\/p>\n<\/li>\n<li data-start=\"3275\" data-end=\"3401\">\n<p data-start=\"3277\" data-end=\"3401\">Families will then benefit from a combination of in-home respite hours and virtual nursing support available 24\/7.<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"3403\" data-end=\"3531\">This initiative also provides additional support during post-hospital transitions, when families often need the most help.<\/p>\n<h2 data-start=\"3538\" data-end=\"3582\">Supporting Dementia Care in Yorba Linda<\/h2>\n<p data-start=\"3584\" data-end=\"3831\">At A Place At Home Yorba Linda, our mission has always been to care for the whole family, not just the condition.
