A Place At Home started out of our founders’ own needs for support while navigating the uncertainty and anxiety of caring for aging loved ones. That’s why we’re so committed to providing excellent, consistent, and comprehensive at-home care to families throughout the Southwest Valley area, including Phoenix, Goodyear, and the surrounding communities.
Meet Our Owner
Don Horner
Owner
Don Horner is a Marine Corps veteran and third-generation Phoenician. His passion for senior care was ignited by his own experience caring for his mother, who lived with Alzheimer’s. This personal journey opened his eyes to the challenges families face and inspired his commitment to providing compassionate, personalized care. Don is especially dedicated to ensuring that veterans, like his father—who is in his 90s and still lives independently—receive the care they deserve. His father’s desire to remain at home drives Don’s mission to help others live comfortably and with dignity.
Outside of his work, Don enjoys staying active by playing pickleball, scuba diving, and coaching sports. These activities help him stay active and connected to his community.

Zylania Parkins
Operations Manager
Zylania Parkins is a strategic leader in healthcare operations and business development, with over 17 years of specialized experience in senior care. Her career has spanned Skilled Nursing Facilities (SNFs), Home Health agencies, and the development of home care startups and franchise models, where she has consistently driven organizational growth, operational efficiency, and clinical excellence.
With a deep-rooted passion for elder care shaped by a family legacy in nursing and office administration, Zylania is known for cultivating high-performing, compassionate care environments. Her leadership is defined by an unwavering commitment to quality, compliance, and the creation of professional, supportive workplaces that empower caregivers and elevate client outcomes.
Originally from New Mexico and now proudly rooted in the Valley for the past five years, Zylania remains actively engaged in the local community. Outside of her professional endeavors, she is a devoted dog mom and a home chef who finds joy in cooking and connecting through food.
